What Is Fibrosis After Liposuction?
By Dr. Mitchell Cohen

One of the most common concerns patients have during recovery is feeling firm areas, lumps, or areas of hardness beneath the skin. In many cases, this is a normal part of healing known as fibrosis.
Fibrosis is the body's natural healing response after liposuction. As the tissues heal, the body produces collagen and scar tissue within the treated areas. This can create temporary firmness, thickening, or a slightly uneven feeling beneath the skin.
Many patients become concerned when they feel these areas a few weeks after surgery. Fortunately, fibrosis is usually a normal part of the recovery process and does not necessarily mean there is a problem.
Fibrosis most commonly develops between two and eight weeks after liposuction. During this period, patients may notice areas that feel firm, lumpy, or tight. These changes often become more noticeable as swelling begins to improve.
How to Minimize Fibrosis
One of the most important things you can do is wear your compression garment as directed. Compression helps reduce swelling, supports the healing tissues, and may help decrease the development of excessive fibrosis.
I also recommend postoperative lymphatic massage. In my practice, patients typically begin lymphatic massage approximately one week after surgery. I generally recommend a minimum of once per week, although twice per week is often even better during the early stages of recovery. Most patients continue lymphatic massage for approximately four to six weeks.
Fibrosis vs. Swelling
It is important to understand that fibrosis is different from swelling. Swelling is caused by fluid accumulation, while fibrosis is the development of collagen and scar tissue within the healing tissues. Many patients experience a combination of both during recovery.
Most fibrosis is temporary and gradually softens with time. In some patients, improvement continues for many months after surgery. One of the most common mistakes patients make is judging their final result too early. Areas that feel firm during the early stages of healing often soften significantly as recovery progresses.
Patience, proper compression, lymphatic massage, and consistent follow-up care all play an important role in achieving the best possible result.
